Dr. Koziarski is an FAA certified Medical Examiner. He is qualified to give both, class 2 and class 3 certificates as well as Student Pilot Medical Certificates. He has been a pilot for over 30 years and currently holds a private license with multi engine instrument and seaplane ratings.
The FAA now requires all applicants to fill out the 8500-8 form (application) online using MedXPress (https://medxpress.faa.gov/). (Please include this hyperlink) This needs to be finished before you come to the office. Please remember to bring your confirmation number with you. If you do not have access to a computer, please plan on coming in the office an hour early, and you can use our computer to fill out the form. You will need the names and dosages of all medications, the names and addresses of any physician appointments in the last three years.
1. Arrive well rested. Try to avoid excessive caffeine, sugar, alcohol or large meals before the exam.
2. You will need to give a urine sample for the exam. Be well hydrated.
3. If you wear glasses, bring them with you.
4. If you have a waiver or special issuance, bring the associated paperwork with you. This is required by the FAA. Please read the letter carefully and bring all documents that the letter specifies with you. Failure to do so may delay issuing your certificate.
